Charles Perrow was an American sociologist known for research on organizations, technology, and risk. In Normal Accidents, first published in 1984, he argued that systems combining complex interactions with tight coupling are inherently vulnerable to unexpected, cascading failures. His framework has been applied to nuclear power, chemical plants, transportation, and other technologies in which small faults can interact faster than operators can understand or contain them.
