Is Ii-chan the same narrator as in Kubikiri Cycle?

Yes, Kubishime Romanticist is the second entry in NisiOisiN's Zaregoto series, and Ii-chan is the same cynical, self-deprecating narrator introduced in the first book, Kubikiri Cycle, alongside the genius Kunagisa Tomo. Del Rey published English editions of the first two Zaregoto novels in the late 2000s.

Who is the killer Zerozaki and does he appear in other books?

This volume introduces the hitman Zerozaki Hitoshiki, whose encounter with Ii-chan became popular enough to spawn an entire spinoff, the Ningen series, centered on the Zerozaki clan of killers. It is one of the main ways Zaregoto connects to NisiOisiN's larger universe.

Is there an anime of this book?

Not of this volume specifically. SHAFT produced an OVA series in 2016-2017 adapting the first Zaregoto novel, Kubikiri Cycle, but the second book's story has not been animated.

Do I need to read book one before Kubishime Romanticist?

Yes, you should, because the book leans heavily on Ii-chan's established personality, his relationship with Kunagisa, and the fallout from the events of Kubikiri Cycle. Starting here would spoil major character dynamics from the first novel.
